Актуальность. Феномен восприятия болезни интересует современных специалистов в области психогастроэнтерологии в силу того, что он во многом определяет ход и результат терапевтических вмешательств. Перспективной линией развития исследований в этом направлении является изучение того, как восприятие болезни, ее причин и последствий связано с соматическим и психологическим дистрессом у людей с болезнями органов пищеварения.
Цель. Изучить восприятие болезни у людей с болезнями органов пищеварения и его взаимосвязей с симптомами соматического и психологического дистресса.
Выборка. В исследовании приняли участие 138 людей, являющихся участниками онлайн-групп пациентов с болезнями органов пищеварения и медицинских форумов с консультациями гастроэнтерологов. Участники исследования были совершеннолетними и имели по крайней мере одну болезнь органов пищеварения, подтвержденную на очном приеме гастроэнтеролога (гастрит, панкреатит, холецистит и др.).
Методы. Для тестирования была составлена анкета, содержащая опросник восприятия болезни (Revised Illness Perception Questionnaire, RIPQ), шкалу соматических симптомов (Somatic Symptom Scale-8, SSS-8), шкалу генерализованного тревожного расстройства (Generalized Anxiety Disorder-7, GAD-7) и опросник здоровья пациента (Patient Health Questionnaire-9, PHQ-9).
Результаты. Восприятие болезни оказалось тесно связанным с симптомами соматического и психологического дистресса у людей с болезнями органов пищеварения: более позитивное восприятие болезни было связано с меньшей отягощенностью соматической, тревожной и депрессивной симптоматологией; напротив, более негативное восприятие болезни было связано с большей отягощенностью соматическими, тревожными и депрессивными проявлениями. Характер протекания дистресса оказался определяющим склонность людей к соматическим и психологическим атрибуциям: при соматическом дистрессе люди были склонны «соматизировать» причины болезней органов пищеварения, связывая развитие болезни с потерей иммунитета, при психологическом дистрессе люди были склонны «психологизировать» причины болезней органов пищеварения, объясняя развитие болезни стрессом, тревогой, напряжением и эмоциональным состоянием.
Выводы. Значительная роль восприятия болезни в соматическом и психологическом дистрессе у людей с болезнями
органов пищеварения определяет важность разработки профилактических и терапевтических вмешательств для развития
и поддержания более позитивного восприятия людьми своих хронических заболеваний.
